When you completed the final simulation in VTRA we gave you a Certificate Confirmation Code. We also showed you the date you completed the course and the name you entered when you started the course. (This information is posted on the bulletin board outside the Hazmat Learning Lab.) That date and that form of your name are associated with your Confirmation Code, so you must must enter them here exactly as we showed them to you.
